HeartAware is a quick, 7-minute questionnaire that is designed to determine your risk factors for developing heart disease which can lead to a heart attack or stroke.
How do I start? To begin the screening Click Here for a free HeartAware Risk Evaluation will open in a separate window. At the end of the assessment, you will be asked if you would like a physician referral or to register for a screening. If you agree (and only if you agree), you will receive the follow-up options.
Is participation confidential? Yes. All information is between you and the hospital.
What if the evaluation shows I’m at risk for cardiac disease? You should print out the results and take them to your physician. If you would like a physician referral you can call 1-800-813-4345 and to learn about our screenings.
What screenings are you currently offering? We are offering a heat health screening that consists of a blood test to determine glucose and cholesterol, electrocardiogram (EKG) a test that measures the electrical activity of the heartbeat and blood pressure test. The clinician will also ask you about your medical history as it pertains to cardiac care. A Cardiologist will intrepid the results of your tests and they will be mailed to you and your physician.
