Request Medical Records

At Lakewood Regional Medical Center, we are required to keep your health care information private. To receive copies of your medical records and images, you will need to complete an Authorization to Release Information form. The form must be filled out completely and can be returned either by fax or by mail. Once received, records will be processed and sent out as indicated on the completed form.
